Dr. Daniel E. Rousso just returned from two meetings recently, the American Academy of Facial Plastic & Reconstructive Surgery (AAFPRS) and the International Society of Hair Restoration Surgery.  

While at the AAFPRS annual fall meeting held in Phoenix, Arizona in October, Dr. Rousso moderated a panel that discussed brow lifting, blepharoplasty, laser resurfacing, and mid-face lifting. These discussions were directed toward helping young facial plastic surgeons, who had been in practice for a few years and were very interested in enhancing their knowledge and learning new technical skills. These session discussions were of immense help to those attending.

While there, Dr. Rousso attended the Fellowship Directors Luncheon, the 1887 Members Luncheon, and the Past Presidents Dinner. Dr. Rousso served as president in 2009.

The annual fall meeting of the International Society of Hair Restoration Surgery was held in Prague, Czech Republic, also in October, where Dr. Rousso was invited to attend The ISHRS President’s Leadership Reception. In celebration of the organization’s Silver Jubilee, Dr. Rousso was recognized for his contributions to hair restoration and was one of the honorees to receive a plaque exemplifying the Society’s three pillars: education, research, and collegiality.

Dr. Rousso attended several vital workshops to enhance his expertise and learn more about the latest techniques in the numerous hair procedures that he performs.

These educational, organizational meetings are invaluable and continue to keep Dr. Rousso on the forefront of the latest techniques in facial plastic & reconstructive surgery and hair restoration surgery.

Plasma Skin Resurfacing

Plasma skin resurfacing (PSR) is a unique technology that can dramatically improve skin quality. The technology has been available for over ten years now, and treatment with our Portrait® non-laser continues to be a viable option for our patients with fine lines, wrinkles, and skin pigmentation associated with photoaging. This FDA-approved procedure delivers millisecond pulses of nitrogen-based plasma energy beneath the outermost layer of skin to stimulate the regeneration of healthy skin.

One of the advantages to PSR is that there is no pain postoperatively. Unlike laser skin resurfacing that removes the outer layer of skin, the skin to remains intact and acts as a biological dressing to minimize discomfort postoperatively.

Several days after treatment, the outer layer of skin will begin to flake off with continued shedding for a few days. After about seven to ten days, patients will feel a softening of their skin. Eyelid Surgery for a More Youthful Appearance

The Differences Between a Mini Facelift and a Traditional Facelift

Before and After Traditional FaceliftFacial aging is inevitable, but not all individuals age in the same way. Differences in facial aging have led to the development of different facelift procedures. Dr. Rousso has helped thousands of patients achieve natural looking, youthful results with both the traditional facelift and the mini facelift. Here are the differences between these procedures to help you choose which technique is best for your needs.

Traditional Facelift

The traditional facelift focuses on major correction of deep facial wrinkles, sagging skin, and muscle bands on the neck. This procedure tightens and removes excess skin and tightens the muscle layer under the skin of the cheeks and the neck. It also lifts the forehead and brows. Muscle bands on the neck are trimmed or tightened, and fat deposits in the jowls and neck are removed with liposuction. All of this is made possible with incisions around the ears, minor incisions hidden in the hairline, and a small incision under the chin.

The traditional facelift is best for patients over age 55 who have severe skin laxity and wrinkles. Dr. Rousso avoids the “stretched” or “operated-on” look and uses endoscopic techniques to minimize scarring and speed your recovery. Recovery from the traditional facelift lasts two weeks and includes no driving, after which patients can resume normal work and activities. The result of a traditional facelift is a natural, rested, and more youthful appearance.

Mini Facelift

Because of the increasing popularity of minimally invasive procedures with fewer incisions and less downtime, the mini facelift uses a minimally invasive technique for more minor corrections. It is best suited for patients who are under age 55 and have only mild to moderate cheek and neck laxity as well as under 55 mm of excess skin beneath the chin. These younger patients are less likely to need the more aggressive technique of a traditional facelift.

With the mini facelift, the facial tissues are elevated and excised as necessary, with limited SMAS undermining. Some of the key advantages of the mini facelift are that it is a less invasive procedure, and it eliminates the mastoid and occipital scars. Additionally, the mini facelift produces excellent cheek and jowl elevation while also addressing moderate submental tissue laxity. The mini facelift can achieve highly positive results and minimize signs of facial aging while eliminating some of the scarring caused by traditional facelift surgery. The recovery from a mini facelift lasts from seven to 10 days and includes no driving. As with the traditional facelift, the result of a mini facelift is a natural, rested, and more youthful appearance.

Rhinoplasty Recovery Guide

Thoughts about a rhinoplasty can begin at a young age. Teenagers concerned about their overall appearance tend to scrutinize their facial appearance. Because the nose is central to the face, if it appears too large or too small, it may not be in proportion to other facial features and may not fit the face. Teens want to feel more self-confident about how they look and how their peers perceive them. Adults who have grown up not liking their nose but not having had the opportunity to change it when they were younger may contemplate rhinoplasty since their circumstances have changed and they now are in a position to have a rhinoplasty.

Functional Rhinoplasty

Teenagers, as well as adults, may consider rhinoplasty due to a traumatic event while playing sports, an auto accident, or other trauma to their nose. A nasal trauma will trigger a trip to the Emergency Room or a doctor for repair. A fracture causing internal injury of the nose can also cause an external injury to the nose making it crooked. Breathing through the nose is usually significantly impaired. This type of event may be covered by the patient’s insurance if deemed medically necessary.

Choosing a Surgeon

Once a patient makes the decision to pursue rhinoplasty surgery it is important that they take the time to research potential surgeons. Important considerations should be the surgeon’s board certifications, expertise, the number of rhinoplasties performed, reputation, and online reviews. Once you select a physician, you will schedule a consultation appointment to meet the doctor and discuss your concerns and goals. At this visit, you should request to see examples of the doctors before and after surgery photos. All of this information will be helpful in determining who you want to perform your surgery.

Getting Ready for Rhinoplasty Surgery

The surgeon will want to know about your past and present medical and social health history. He/she will want to know if you have a medical condition that you are currently being treated for, smoke, consume alcohol, or take medications that may be contraindicated to your having surgery. Patients are advised during their consultation with the doctor if they believe they are a candidate for the proposed rhinoplasty surgery. We require our patients to have a medical clearance from their primary doctor, an EKG (over 39), and lab work. After receiving the necessary medical clearance reports and labs obtained prior to a surgery date, the patient will be ready for surgery.

After Surgery

The surgery can be performed here in our clinic or at the hospital. Patients and the physician determine where the surgery will be performed based on several factors. After surgery, the patient will recover at the Embassy Suites with a qualified Sitter trained in Dr. Rousso’s postoperative protocol. There is usually very little discomfort following a rhinoplasty that we perform. We do not routinely pack the nose following a rhinoplasty, and that makes the recovery process much more comfortable. Rhinoplasty patients will require cool ice packs every 20 minutes to their eyes and nose post-surgery to help keep swelling and bruising to a minimum for 24 hours. The patient will sleep at a 30-degree angle that night to help reduce swelling and continue that for the next couple of weeks. The patient will be seen in the office the next morning after surgery by the doctor. The patient should not blow their nose immediately after surgery and for the first week. They will also want to be careful not to get bumped while recovering. Patients’ are required to take it easy for the next few weeks. They will return to see the doctor one week after surgery to have the nasal splints removed. After that, they will again return at two weeks to check on how the healing process is going. The majority of swelling (80%) will be gone at two weeks. The remainder will dissipate over the next several months but will not be noticeable. It is important for the patient to continue to follow postoperative instructions to continue the healing process at home which includes rest, not lifting more than 10 pounds and limited physical activity for six weeks.

Patients who suffer from allergic reactions to lidocaine should not undergo Restylane® Silk treatments. It is important for patients to discuss possible allergies to medications and other substances with Dr. Rousso.

Recovery and Results

All facial fillers may result in some mild side effects, including bruising, redness, and swelling at the injection site. These side effects are very temporary and subside after a few hours.

Facial fillers are selected based on the site to be treated and on which will offer the most optimal result. The number of treatments required to achieve a patient’s desired cosmetic goal will differ, as will the longevity of the results, based on the selected filler. One can expect that most fillers will need to be repeated at 6 to 12 months.
